We've recently had a series of failures when making bread with our Panasonic breadmaker - the bread fails to rise and comes out like a house brick.

After reading this article, It seems highly likely that the cause is the yeast. As many people mentioned, the yeast seems to be the cause of this problem, even if it's within the sell-by date. A couple of extra clues: The failing yeast had a rather nasty smell, nothing like the pleasant yeasty smell.
